Welcome to Shawbirch Medical Centre

 

 

Shawbirch Medical Centre has been looking after the people of Shawbirch and Wellington since 1989. We aim to offer an efficient and high quality service, as well as a friendly atmosphere for our patients and staff. To ensure that this occurs we need and value your opinion, so please bring any problems you experience to our attention as soon as possible.

 

Disabled Access


Shawbirch Medical Practice has plenty of car parking outside the building, and all consulting rooms are on ground level. We have disabled toilet facilities within the building.

 

 

Patient rights and responsibilities


We try our hardest to offer you a high standard of care, but in order for the service to run efficiently we have to make some requests of yourselves.

 

We need you to ensure that you keep the appointments offered to you and follow the medical advice given to you. If you are unable to attend for any reason, please let us know as soon as possible so that we may be able to release that appointment for another patient.

 

Occasionally, the patient-practice relationship breaks down to a point where the patient may choose to register elsewhere. Please note that the practice also has the right to remove a patient from their list under certain circumstances.

 

Please note the NHS operates a zero tolerance policy towards violent or abusive patients. We have the right to remove violent patients with immediate effect. In this situation we will write to the patient advising them of their removal, and record this in their medical notes.

 

Confidentiality


All of our staff are aware of their duty to you to keep your medical information confidential. The information we hold will not be used or released for any other purpose than for your clinical care. The only times we would disclose information would be with your consent, if the law required it, or if there were an overriding need to society. We consider the practice a safe haven for your medical records.  We intend to issue reminders to the patients of follow-up appointments for certain diseases (e.g Asthma, Diabetes, Circulatory disease etc.) by e-mail in future, if you have any concerns about this please discuss with practice manager.

 

 Complaints procedure


A high standard of care is one of the priorities of this practice and we welcome your comments as to how we might improve the service. If you have any comments or criticisms please contact the Practice Manager. Problems of an administrative nature will be investigated initially by the Practice Manager, medical problems by the senior partner. If the problem is not resolved to your satisfaction we will advise you how to pursue the matter via Telford and Wrekin Primary Care Trust.
