You do not have to be a newcomer to the area to register with our surgery. If you are considering changing from your current practice, and fall within our practice boundaries, we would be happy to hear from you.
Registering at the Surgery
Patients wishing to register with our practice may approach our receptionists who will guide them through the simple process of registration. You will be asked to complete a registration form and will be offered an appointment with one of our health care assistants. (initial tests)
Your medical records, which are held at your previous surgery, will be requested by the Health Authority and sent on to us so that we can update our own records.
Under the new NHS arrangement, you are registered with the practice, not with a specific doctor. However, you are free to consult with the doctor of your choice, provided they have an appointment available.
Since April 2004 the Primary Care Trust has advised that practices should define a 'practice boundary' for patient acceptances. Our practice area covers most of the North Telford Area.

Temporary Residents
Should you have a friend or relative staying with you from out of the area, who requires medical attention during their stay with you, you may register them with the surgery as a temporary resident. A temporary resident form will need to be completed before you can book an appointment.

**Newly registered patients will be called for a routine new patient medical with a member of our nursing staff. During this examination your blood pressure and urine will be tested, and a note of your past medical history and current medication will be taken.
